I have been informed that my personal data may be processed both manually and by automated means, including through searches and/or access based on specific criteria (e.g., files, lists, databases, logs, etc.). 

I have been informed of my rights regarding the processing of personal data as outlined in Chapter 3 of the Law.

1) I have the right to obtain information regarding the processing of my personal data. This means I can, free of charge, learn the name and location of the Operator and its authorized representatives, obtain confirmation that my personal data is being processed, and find out the specific personal data being processed, including the sources from which it was obtained, the legal basis for processing, the purposes of the processing, and the processing periods. I am not required to justify my interest in the requested information;

2) I have the right to request the rectification of inaccurate personal data. This means that if my personal data is incomplete, outdated, or incorrect, I can submit a request accompanied by relevant documents or certified copies thereof to confirm the need for changes;

3) I have the right to obtain information about the disclosure of my personal data to third parties. This means I can access information about all instances where my personal data has been disclosed to third parties within the year preceding the date of the request. Unless otherwise provided by law, exercising this right (no more than once per calendar year) is free of charge;

4) I have the right to request that the processing of my personal data be stopped or that my personal data be erased. This means I can demand that the Operator cease the processing or delete my personal data (unless there are other legal grounds for processing as provided for by law). If it is technically impossible to delete the personal data, the Operator shall take all necessary measures to prevent its further processing;

5) I have the right to withdraw my consent to the processing of my personal data. This means that upon receipt of a notification from me withdrawing my consent, the Operator shall cease processing the personal data for which I originally gave my consent;

6) I have the right to lodge a complaint against the Operator for any actions (or inaction) that violate my rights in relation to the processing of my personal data. I can submit a complaint to the authorized supervisory authority for the protection of personal data (the National Data Protection Center of the Republic of Belarus) in accordance with the procedure established by the legislation on citizens' appeals.

Regardless of the form and method of exercising these rights, such a statement must contain at least the following information: first name, last name, patronymic (if any), residential address (or temporary residence), date of birth, a statement of the claim, identification number, or if such a number is absent, the number of an identity document, in cases where this information was provided when giving consent; personal signature.

The Operator does not consider statements from data subjects sent in other ways (e-mail, telephone, fax, etc.).

A properly filed application will be reviewed by the Operator within the time frame specified by law, and I will be notified of the decision.

The right to appeal the actions (inactions) and decisions of the Operator related to the processing of personal data is carried out in accordance with the procedure established by the legislation of the Republic of Belarus on appeals from citizens and legal entities.

I have been informed that by giving my consent to the processing of my personal data, I grant the Operator the right to perform actions related to the processing of personal data, including collection, systematization, storage, modification, use, anonymization, blocking, provision, and erasure of personal data.

Refusal to give or withdrawal of consent to the processing of personal data will result in the impossibility of further processing of personal data for the purposes of processing, the cessation of processing of personal data, their erasure, and, in the absence of technical capability – their blocking.
